Important Information for Tax Preparation

Please fill out this form with as much information as possible. It is necessary that for your appointment you bring certain documents such as:

INCOME: W2 Forms, Bank Statements, Social Security income statements, Unemploymeny Stubs, IRA's.

EXPENSES: Mortgage interest statement, and Real Estate [property] taxes. Letter or statement from a church or other entity where you made yearly contributions over $250.00. Statement from a day-care facility showing the entity's name, full address, federal ID number, and amount paid for the year.

Please bring the social security cards of all the individuals that appear on your income tax.

I f you are jointly filing with your spouse, both parties should come into the office to sign the documents.
